Outline the scope of work to be paid for with City of Loveland Grant Funds.  Completed scopes of services will be included in all grant contracts.

CDBG Bricks and Mortar scopes of services are slightly different.

This form is used to update the levels of service projected in the grant application.  Generally, updates are proportionate to the amount requested : amount awarded ratio.

Drawing Down Funds

Current grantees must submit a drawdown form along with sufficient documentation of expenditures prior to the release of funds.

Disbursement of Funds Schedule

Drawdown requests are processed by the Community Partnership Office on the 1st and 3rd Fridays of the month.  Funds are disbursed to agencies the Friday after the request was processed.

2009 HUD Income Guidelines

A minimum of 70% of those served by the Human Services Grant and CDBG Programs will be of low income as per HUD guidelines. 

 

Human Services Grant Recipients must report project activity at time of final draw down, or by June 30, 2010.  Download the Final Report Form.

CDBG Recipients must report project activity on a quarterly basis, per HUD guidelines. Quarterly Reports are due to the Human Services Office on the following dates:
January 15                                 
April 15
July 15                                         
September 15
